DOG LICENSE FORM
Kentucky Humane Society
4667 N. Main Street
Eminence, Ky. 40019 845-6446
Eminence Residents,
It is time again to license your dogs in Eminence. .You may license them from 8:30 am to 3:30 pm at the Kentucky Humane Society Office (4667 N. Main Street) Monday thru Friday using the form at the bottom of this letter. If yon prefer, you may also send it by mail (please enclose copies of your certificates of spay/neuter and rabies vaccination).
Please keep your animals on a leash and do not allow them to run loose in the street. We have had many complaints about roaming animals. Please be aware that they will be impounded and a citation issued if they are found doing so. The current practice is that the first time an animal is picked up only a verbal warning is issued.
Without a current license attached to a collar, it is almost impossible to determine ownership of a dog, and sadly enough they usually end up at the shelter. Please attach the license to your dog's collar as soon as you get it.
Licenses are now available for 2009.
$5.00 for altered animal $10.00 for unaltered
